Season 12 "Slaughter" has been live for a while, but here's the hard truth: most Bloodied Sigils you're picking up are worthless.
Not “suboptimal” — straight-up dead rolls that won't carry you past mid-tier Slaughterhouses. And the game never tells you which ones are actually good.
This guide breaks down the top affix rankings and most efficient farming methods for Bloodied Sigils.
Before chasing the perfect affixes, let's go over the S12 core mechanic.
Bloodied Sigils are special keys introduced in Season 12. They can be applied to Nightmare Dungeons, Helltides, and Lair Bosses. Unlike normal sigils, every Bloodied Sigil comes with a fixed affix: Relentless Butcher.
This means the Butcher is no longer a random surprise. He will hunt you non-stop, and when killed, he respawns shortly after – keeping up the pressure. In some extreme cases, you might even face two Butchers at once.
Using a Bloodied Sigil guarantees Bloodied Items drops, and the affixes on those items scale with your kill streak. High risk, high reward.

Bloodied affixes are split into three gear slots:
Rampage (Armor) – Power scaling based on current kill streak tier
Feast (Weapons) – Combat bonuses based on total kills in the streak
Hunger (Jewelry) – Drop adjustments that scale with streak tier
Max roll: +6% per streak stack (values shown are for 925 Item Power)
The single most valuable affix in the current meta. More ults = more invincibility/up-time. With high CDR, you're a god; without it, you're a corpse. Works for every build. Especially impactful for Barbarians relying on "Marshall" or "Earthquake" loops, and Necromancers needing frequent ults.
Max roll: +8% per streak stack
8% per stack easily pushes you past weapon breakpoints. Huge DPS and QoL boost. Faster attack speed = faster animation canceling, more Life on Hit triggers, and faster streak building. Universal. A game-changer for Rogues, Barbarians, and Spiritborn who rely on core skills or basic attacks.
Max roll: +3% per streak stack (Strength/Dexterity/Intelligence/Willpower – auto-adapts to your class)
The dark horse buffed in S12 patch. Core stat has its own multiplicative bucket in D4's damage formula. At high streaks, this is like wearing two extra perfect-rolled ancient items. The best raw damage affix for late-game pushing. Universal. Mandatory for high-tier pushing.
Max roll: +8% per streak stack
+8% crit massively relieves gear affix pressure. Lets you swap crit rolls on gear for crit damage or max life. Easily hit 100% crit and trigger all "on crit" aspects/passives. Universal for crit builds. Top priority for Rogues, Necromancers, and Sorcerers.
Not as busted as S-tier, but still solid. If you don't have S-tier, these are your go-to.
Max roll: +10% per streak stack
+10% resource reduction means you can drop your basic skill and spam core skills non-stop. The key to breaking energy loops – a "perpetual motion machine." For builds that starve for resource: Bone Spirit Necro, Whirlwind Barb, Lightning Sorc.
Max roll: +3% per streak stack
In high tiers, that extra health often turns a one-shot into a near-death experience, buying time for potion CD and skill rotations. Lower damage than S-tier, but it's your survivability insurance. Dead = no DPS. Universal. Prioritize when pushing high tiers.
Max roll: +X per streak stack (scales with item power)
Paired with high attack speed, this enables face-tanking. Multiple heals per second = a mini healing spring. Essential for builds lacking sustain. Great for high-AS builds: dual-wield Rogues, Whirlwind Barbs, snapshot Spiritborn.
These can shine in specific builds, but are less universal. Use them if you have nothing better, but don't chase them.
Max roll: +10% per streak stack
+10% move speed feels great for dodging Butcher hooks, fast farming, and kiting. Improves QoL and survivability (positioning). Near-zero value during boss DPS phases. Nice to have, but not run-changing. Transitional use.
These look flashy but are noob traps. If you see any of these, salvage – no exceptions.
Overpower Damage – Extremely low proc rate (only certain skills or 3% base). Useless unless you're a very specific Overpower Necro (Blood Bath). Trash. Salvage.
Thorns Damage – Scaling is terrible. Mobs in high tiers have hundreds of millions of HP; thorns tickles. Useless outside a meme thorns build. Salvage.
Flat-value affixes (not "% per streak") – Completely outscaled by per-streak % affixes. A relic from past seasons. Salvage.
Complete specific Season Journey chapters to earn Greater Bloodied Caches – they guarantee sigil drops. Additionally, you can earn Brutality currency from kill streaks and trade it for more caches in the seasonal menu, creating a self-sustaining loop.
Mystery Chests in Helltide have a high chance to drop Bloodied Sigils. Focus on clearing Helltide towers (Flame/Soul Towers) – hot spots have better drop rates. Helltide is also great for farming Fresh Meat to spend at the Butcher vendor in Kyovashad.
Nightmare Dungeons and Helltide final chests can directly drop Bloodied Sigils. Higher torment tiers = better drop rates. Torment III and above are your best bet for rare rolls.
Once you have sigil powder and gold, visit the Occultist to craft and upgrade Bloodied Sigils. Upgrading is 100% safe – only costs powder and gold.
Unlock : Reach level 100 and complete all Season Journey chapters.
Crafting cost (example) : Tier 1 Bloodied Sigil costs 800 powder + 60,000 gold. Each Zir Slaughterhouse tier you clear unlocks the next recipe.
Tip: Higher-tier sigils cost more. Start from low tiers and work your way up.
Streak is everything – Bloodied Sigils and Bloodied Items both scale with your kill streak. Keep it going. Dying instantly resets your streak to zero.
Streak tiers: Killstreak (0-99) → Carnage (100-249) → Devastation (250-499) → Bloodbath (500-999) → Massacre (1000-9000) → Fresh Meat! (9000+)
When you see "Fresh Meat!" pop up, you know you've reached peak Slaughter.
Drop difficulty for XP farming – If you're farming XP instead of pushing, drop 2–3 Torment tiers before using a Bloodied Sigil. You'll kill the Butcher faster and get way better XP/hour.
Group up for safety – The relentless Butcher pressure is much easier to handle in a group. Split aggro, chain CC, and boost your survival rate significantly.
Focus on S-tier combos – When chasing perfect sigils, prioritize any combination of CDR / Attack Speed / Core Stat / Crit Chance. One S-tier affix = good base. Two S-tier affixes = best-in-slot.
Farm Helltide on weekends – Weekend Helltides + Bloodied Sigils = double dip on drops. Stockpile your sigils for weekend sessions.
CDR / Attack Speed / Core Stat / Crit – those are your S-tier gods. Get one and you're solid. Get two and you're pushing leaderboards.
See Overpower, Thorns, or flat-value (non-streak) affixes? Salvage without a second thought.
