Season 12 doesn't introduce a brand-new endgame activity. Instead, Blizzard is changing how existing content feels. Nightmare Dungeons, Infernal Hordes, and Lair Bosses are still the core of Diablo 4's endgame, but with Bloodied Sigils, those runs no longer feel routine.
If endgame content has started to feel predictable or too safe, Season 12 is designed to push back — not with more systems, but with more pressure.

Bloodied Sigils are special modifiers you apply directly to content you already play:
There are no new queues and no separate modes. Activate a Bloodied Sigil, enter the run as usual, and the difficulty immediately spikes. The reason is simple: every Bloodied Sigil comes with Relentless Butcher.
With Relentless Butcher active, the Butcher is no longer a random encounter.
In rare cases, the standard Butcher can also appear at the same time, creating double-Butcher pressure. There's no slow clearing and no safe downtime. From start to finish, the run stays aggressive.
This single modifier completely changes endgame pacing.
Bloodied Sigils are built around two other Season 12 systems:
The return of Killstreaks
The introduction of Bloodied Items
Together, they reward players who stay alive and keep fighting. Playing slowly is safer, but it's no longer optimal.
As you defeat enemies, you build a Killstreak that slowly drains over time. To maintain it, you must keep dealing damage and securing kills.
Killstreak tiers progress as follows:
When a Killstreak ends naturally, you earn bonus Experience and seasonal reputation based on:
Total enemies killed
Highest tier reached
If you die, the Killstreak ends immediately — with no rewards.
Survival and momentum now go hand in hand.
Bloodied Items are standard items with special affixes that scale while your Killstreak is active. They don't replace Ancestral gear and can stack with other affixes.
Bloodied affixes fall into three categories:
Scales with your current Killstreak Tier, offering bonuses like Movement Speed or Cooldown Reduction.
Triggers bonuses based on total kills, such as resetting random cooldowns or granting Berserking.
Improves reward drops during Killstreaks, including increased Rune drops from Elite enemies.
The longer you stay alive and keep killing, the stronger your character becomes during the run.

Bloodied Sigils don't add new content — they change how existing content is played.
Playing safe still works, but the best rewards go to players who can stay aggressive under pressure without dying.
Season 12 refreshes the endgame by increasing tension rather than adding layers. Familiar activities feel dangerous again, and rewards are earned through survival, not patience.
Whether Bloodied Sigils feel exciting or exhausting will depend on the player. But one thing is clear: Diablo 4's endgame isn't getting bigger — it's getting sharper.
