Because of this, many farming builds run Hunger jewelry to improve overall loot efficiency.
Bloodied items are not a separate rarity either — they can also drop as Ancestral gear, which means they remain useful even in high Torment tiers.
Bloodied items start appearing once you reach Torment 1 difficulty.
The most common sources include:
Bloodied Sigil dungeons
Nightmare dungeons
Infernal Hordes
Lair Boss encounters
Bloodied Sigils are especially important because they add a special dungeon modifier called Relentless Butcher. This modifier increases difficulty and spawns a Butcher that constantly hunts you during the run.
The upside is simple:
Completing a Bloodied Sigil guarantees a Bloodied item drop.
If your goal is to build a full Bloodied setup quickly, this route works consistently in Season 12.
Bloodied Sigils and better gear rolls only begin appearing after Torment 1.
Before that, focus on:
leveling builds
seasonal quest progress
unlocking endgame systems
Once Torment difficulty opens, Bloodied farming becomes much more efficient.
Killstreak bonuses work best in areas with large monster packs.
Good farming options include:
Nightmare Dungeons with dense mob layouts
Infernal Hordes runs
Helltide farming loops
The key is never letting your kill chain reset.
Killstreaks progress through five tiers:
Killstreak → Carnage → Devastation → Bloodbath → Massacre.
The higher the tier, the stronger your Bloodied affixes become.
Once you obtain Bloodied Sigils, chain-run them.
Typical farming session numbers:
6–8 Sigil runs per hour
8–15 Bloodied item drops per hour

Because Bloodied Sigils guarantee drops, they are currently the most reliable farming method.
With a decent endgame build and efficient routing, most players average:

dozens of crafting materials and runes
Clear speed matters a lot here. Builds that maintain high movement speed and AoE damage can push these numbers even higher.
A typical farming setup mixes all three affix types.

Feast effect to trigger combat bonuses during kill chains

Hunger affixes to increase drops and farming efficiency
This combination allows you to scale both damage and loot income at the same time.
Farming Bloodied gear is fun at first, but a full build usually needs 8–10 pieces with the right affixes.
