Finished Act 3 but never stabbed the heart? The Sacrificial Heart quest still grants free passive points in Path of Exile 2 patch 0.5 — if you know which zone drops it and how to reopen Aggorat.

This guide covers the Act 3 drop route, the Aggorat blood ritual, instance resets, and the fixes players use when the Time Travel Gateway closes before they sacrifice.

Act 3 Past Zones — Why the Heart Still Matters?

The Sacrificial Heart is a quest item, not trade loot. In-game text calls it a soul clinging to fading life. You sacrifice it at a ritual altar in Aggorat to earn permanent character power.

You reach Act 3 past content through the Time Travel Gateway in the Ziggurat Encampment. Utzaal and Aggorat sit in that timeline. The heart drops from Vaal packs there, then converts into skill points at the altar.

Patch 0.5 and Runes of Aldur did not remove this quest. New league characters still want the points before Atlas farming.

 

Where the Sacrificial Heart Drops?

The heart is a random drop from enemies in Utzaal and Aggorat. It is not guaranteed on the first pack.

Utzaal — first past zone

Utzaal is the first area after you step through the gateway. Vaal Goliaths and Vaal Overseers are the packs most players farm. Clear the zone, loot ground drops carefully, and note quest items separately from gear.

If nothing drops, do not abandon the quest. Reset the area instead of pushing Acts 4–5 and hoping the game remembers.

Aggorat — past the Legion Blockade

Aggorat opens after you defeat the Legion Blockade boss in Utzaal. Chanting audio and stairs behind the large statue mark the route in.

Aggorat enemies can also drop the heart. The zone is denser and harder than Utzaal. Bring resistances and flasks before extended farming.

 

Reset Aggorat When the Heart Will Not Drop

Instance resets are the normal farming tool, not an exploit.

Open the world map, hold Ctrl, and left-click Aggorat to create a new instance. Re-enter with fresh packs. Repeat until a heart drops or you choose to continue the campaign and return later.

Waypoints and checkpoints both help navigation, but the Ctrl+click new instance step is what actually refreshes drops. Many players miss the heart simply because they never reset.

 

Complete the Blood Ritual at the Altar

Once you hold a Sacrificial Heart, return to Aggorat and follow the Trail of Sacrifice toward the blood altar. Bleeding corpses and stairs up to a large dais mark the site.

Pick up the Sacrificial Dagger on the altar. Place the heart in the center. With the dagger in your inventory, choose Stab Sacrificial Heart. A short cutscene plays and the heart leaves your bags.

If the option never appears, confirm both items are in your inventory — not on the cursor, not in stash — and that you are at the correct altar in Aggorat, not a random quest object elsewhere.

 

Rewards You Keep After the Ritual

Completing the ritual grants:

  • +2 Passive Skill Points
  • +2 Weapon Set Passive Skill Points

Both are permanent on that character. They persist through Acts 4–6 and into endgame mapping. Spend them on your planned tree rather than random picks — you cannot undo the allocation cheaply later.

These points are separate from Ascendancy points and Book of Specialization rewards. The heart quest is one of the earliest permanent power spikes in Act 3 past content.

 

Missed the Ritual — Gateway Closed After Act 3

This is the most common post-2025 question. You beat Act 3 present timeline, the gateway collapses, and the heart still sits in your inventory.

Try these routes in order:

  1. Return to Act 3 Normal via waypoints. Re-enter the Time Travel Gateway from the Ziggurat Encampment if it is available again.
  2. Right-click the Ziggurat Encampment waypoint on the map and choose to reset the instance. Players report this restores access when the gateway stayed closed after rushing Act 3.
  3. On Cruel difficulty, travel back to Act 3 Normal, pass through the gateway, then advance to Act 1 Cruel town. Open the waypoint map — Aggorat may appear as a direct destination once past zones are re-linked.

If the heart or dagger was sold or dropped, check whether the altar still offers interaction. Some players recover the ritual without re-farming; if not, farm another heart in a fresh Utzaal or Aggorat instance.

 

Cruel Difficulty and a Second Heart

On Cruel, you can loot another Sacrificial Heart and repeat the ritual steps. The permanent passive bonuses apply once per character per the normal quest reward track — do not expect unlimited points from repeating Cruel for the same character.

Treat Cruel as a safety net if you skipped Normal, not as a duplicate stat farm.

 

Before You Leave Act 3 Past Content

Sacrifice the heart before you rush forward if you can. Two passive points and two weapon-set points matter more than shaving ten minutes off campaign time.

If you are racing ladder start and refuse to pause for drops, at least bookmark this route. Returning with a reset instance beats leveling to endgame with a weaker tree.

When campaign friction is currency — not the ritual itself — some players pick up Path of Exile 2 currency through MMOSO to keep mapping while they finish Act 3 cleanup on an alt.
