Diablo 4 Season 12 Pound of Flesh Farm Guide

Hey everyone. If you're still mindlessly farming Helltides in Season of Slaughter (S12), you're probably missing out on the core endgame loop. Today, we're talking about something with a gruesome name, a very simple purpose, and a very specific farming method.

It's called Pound of Flesh — and it's not just a quest item from that one side mission. In Season 12, it's your ticket to the Butcher's treasure room.

What Actually Is Pound of Flesh?

In Diablo 4 Season 12, Pound of Flesh is a Lair Boss Key. Unlike previous seasons where you had to summon bosses with materials, in S12 every Lair Boss can be summoned for free. But here's the catch: after you kill the boss, a Butcher Hoard chest appears on the ground, and you need to spend 3x Pound of Flesh to open it.

In other words: Boss kills are free. Loot is locked behind Pound of Flesh. If you're not farming this item, you're basically skipping rewards.

Where to Farm Pound of Flesh (Best Methods in Diablo 4 S12)

Method 1: Bloodied Activities – Best Pound of Flesh Farm

Bloodied Sigils were introduced in Season 12. You can use them to upgrade Nightmare Dungeons, Infernal Hordes, or Lair Bosses into Bloodied Activities.

Inside these, Pound of Flesh drops frequently. The best modifier to look for is "Relentless Butcher" – this causes the Butcher to repeatedly spawn, and each kill has a high chance to drop Pound of Flesh.

Pro tip: If your only goal is Pound of Flesh, run Bloodied Infernal Hordes. The combination of high mob density and frequent Butcher spawns makes this the best choice for Pound of Flesh farming.

Method 2: Undercity of Kurast (Vessel of Hatred)

If you own Vessel of Hatred, this is one of the best alternative farming routes.

Enter the Undercity with a Tribute of Titans active. Upon completion, you'll receive a large stack of Lair Boss materials, including multiple Pound of Flesh. Community testing shows this is the second-best source overall, just behind Bloodied Activities.

Method 3: Greater Bloodied Caches – Passive Income

In Diablo 4 S12, you earn Brutality points by chaining kills. This unlocks seasonal reputation rewards. After reaching max rank (24), every additional 12,500 Brutality rewards a Greater Bloodied Cache.

Each cache contains:

- Pound of Flesh

- Fresh Meat (another seasonal currency)

- Bloodied Sigils

- Bloodied Lair Keys

- Ancestral Bloodied gear

This isn't a targeted Pound of Flesh farm, but it's a very nice bonus while you do other content.

How to Farm Brutality Efficiently (For Those Caches)

Since Greater Bloodied Caches are a steady source of Pound of Flesh, you want to maximize Brutality gain.

The answer: farm The Pit, specifically Tier 55.

Why T55? The Pit T55 gives the same Brutality as higher tiers, but clears much faster. There is no benefit to pushing higher tiers for Brutality farming.

Brutality values (Torment 4):

- Blue Champion packs (3-5 mobs): 60 each

- Yellow Elites: 150 each

- End boss: 720

The Pit T55 Farming Loop:

1. Enter The Pit T55

2. Prioritize blue Champion packs – they have the highest density

3. Keep your killstreak active for the completion bonus

4. Always finish the boss (don't skip the 720 Brutality)

With a good build, you can clear T55 in under 2 minutes. That's a Greater Bloodied Cache roughly every 6 minutes.

How to Use Pound of Flesh

Once you have at least 3x Pound of Flesh, head to Gea Kul in Kehjistan. Find the entrance to "The Broiler" (The Butcher's lair).

Enter The Broiler and kill The Bloody Butcher. After he dies, a Butcher's Hoard chest appears in the center. Spend 3x Pound of Flesh to open it and collect your loot. This is the only way to turn your farming efforts into actual loot.
