Malice is a Level 15 melee runeword made with Ith + El + Eth. It is cheap to craft and useful for early-game melee characters, especially when you need Open Wounds or a low-cost bossing weapon.

Crafting D2R runewords follows strict fixed rules. Players must use a base weapon with an exact socket count and insert runes in the correct sequence. Any mistake will fail the runeword craft.

Only normal white or superior 3-socket melee weapons work for Malice. No more or fewer sockets are allowed. Early Act 1 and Act 2 melee bases like Flails are perfect beginner options for Barbarians and other melee classes.
All three crafting runes (Ith, El, Eth) are low-level farmable runes. Players can consistently collect these runes by farming the Countess in Normal Act 1. This farming method is highly recommended for melee builds. The Malice runeword's combat bonuses drastically simplify early boss fights and mob clearing.
One critical reminder: Slotted runes cannot be recovered. Always confirm the exact rune order before inserting runes into your base weapon.
Malice provides a balanced mix of damage boosts, enemy debuffs, and anti-healing effects, with one minor trade-off penalty. Every stat serves a clear purpose for early melee combat.
The biggest advantage of Malice is its 100% Open Wounds proc. Open Wounds causes enemies to bleed continuously for 8 seconds. During the bleed duration, targets cannot regenerate health.
This mechanic is overpowered for early and mid-game boss fights. It works perfectly against high-regen bosses like Duriel, Diablo, and Baal. You can also keep Malice on weapon swap if you only want to use its Open Wounds and anti-healing effects against tougher enemies.
Malice includes two separate defense reduction mechanics with different functions.
The -25% Target Defense stat does not permanently lower enemy defense. It increases your hit rate by calculating enemy defense at a 25% reduced value, paired with +50 Attack Rating for higher accuracy.
The -100 Monster Defense per Hit effect stacks with every successful attack. Repeated hits continuously lower enemy defense, making subsequent strikes far more consistent.
The Prevent Monster Heal stat completely blocks natural enemy health regeneration. This eliminates the biggest threat of high-health late Normal and Nightmare bosses.
The only downside is the Drain Life -5 penalty. This stat does not drain 5 HP per second. It only drains roughly 1 HP every two seconds. Players can fully offset this penalty by socketing a Perfect Skull in their helmet.
Not all melee bases perform equally. Different base weapons match specific classes and mercenary builds. Below are the four most optimal Malice bases with clear use cases.

Blade Talons are tailor-made for Assassins. Its high attack speed triggers Open Wounds and defense reduction effects constantly.
Halberds are the best mercenary base. It delivers high damage and long-range attacks, ideal for clearing packs and bosses in early difficulties.
Malice excels on fast-attack melee builds. Rapid strikes trigger all on-hit debuffs and Open Wounds effects to their full potential.
Smiter Paladins specialize in boss killing and Uber content. Malice's Open Wounds and Prevent Monster Heal stats stop boss health regeneration completely. It is a perfect transitional weapon for Act boss farming before end-game gear.
Frenzy Barbarians have extremely high attack speed and single-target damage. Every frenzy swing stacks Malice's defense debuffs, maximizing hit accuracy and bleed damage.
Zealot's multi-hit skill stacks monster defense reduction in seconds. The Paladin's Prayer Aura also offsets Malice's minor life drain penalty, creating a self-sustainable early build.
Kicksin builds do not rely on weapon damage, but they fully proc weapon modifiers. Open Wounds and defense debuffs apply to kick attacks, making Malice an excellent swap weapon for Assassin boss fights.
